Dimpleplasty surgery is a facial plastic and aesthetic surgical procedure which is carried out under local anesthesia, which lasts half an hour, without hospitalization and which does not leave scars. The results of this intervention are generally very satisfactory.
Dimple surgery or dimpleplasty is very widespread in the United States and the Middle East and has been increasing sharply in France in recent years. In many cultures, dimples are a sign of beauty and luck. In the Middle East, Asia and India, dimples are considered a divine sign.

History and symbolism of dimpleplasty
In the Middle East, dimples are considered a sign of beauty, attractiveness and luck.
There is a growing demand for cheek dimple surgery these days.
The presence of dimples is inherited in an autosomal dominant manner. Cheek dimples are expressed on chromosome 16, with variable penetrance. Often parents with dimples will pass it on to their children.
Cheek dimples are caused by the presence of a dermocutaneous insertion of the zygomatic muscle.
Several techniques have been described for creating dimples based on two main principles: excising the soft tissue or using the suture technique or the combination of both.
What is a dimple?
In a person with dimples at birth, these are due to the presence of a dermocutaneous insertion of the fibers of the lower bundle of the zygomaticus major muscle. In other words, natural dimples are due to adhesions between a facial muscle (zygomaticus major) and the skin of the cheeks.
The term “dimpleplasty” refers to dimple surgery, with the aim of aesthetic improvement.
The dimple can be considered as an adhesion of the skin with the oral mucosa. In a subject without dimples, the different planes of the cheek will slide over each other when smiling (skin, muscle, mucous membrane) without adhesion. Dimple surgery will then consist of removing the muscle on a tiny area. In this way, we will create an adhesion between the skin and the mucous membrane and when the subject smiles, the mucous membrane will attract the skin and then create the dimple.
The procedure aims to surgically create natural-looking dimples in the cheeks.
Type of anesthesia: Usually the procedure is performed under local anesthesia on an outpatient basis.
The scars are made inside the mouth and are invisible from the outside.
Dimple location: how to choose it?
The ideal location for marking unilateral or bilateral dimples is the area where a less pronounced dimple is already present with the smile.
If the patient has no pre-existing dimple, it is marked at the intersection of a perpendicular line passing through the outer canthus and a horizontal line drawn from the corner of the mouth (Koo Bai Chai Technique)
Procedure of the dimple surgery operation
Since Dr. Koo Bai-Chai first operated on cheek dimples in 1962, several authors have described different techniques for connecting active facial muscles to the dermis. In 1971, Dr. Argamaso excised the cylindrical segment of tissue consisting of oral mucosa, muscle and subcutaneous fat to form a permanent dimple.
Indeed, suturing alone without tissue excision has no lasting results and the dimple fades over time.
The operation is carried out under local anesthesia. A needle is inserted through the cheek directly above the skin mark. By an intraoral route, by everting the mucosa, a collar of intraoral mucosa, subcutaneous fat and muscle is removed. After careful hemostasis, two sutures are made inside the mouth with 3-0 rapid vicryl thread. Non-transfixing sutures allow the dermis, muscle and intraoral mucosa to be joined together to create the dimple.
What are the possible complications of dimple surgery?
Dimple surgery or Dimpleplasty, although carried out for essentially aesthetic reasons, nevertheless remains a real surgical intervention which involves the risks inherent in any medical procedure, however minimal.
This act remains particularly subject to the hazards linked to living tissues whose reactions are never entirely predictable.
Regarding the surgical procedure: by choosing a quality and competent Plastic Surgeon, trained in this type of intervention, you limit these risks as much as possible, without however completely eliminating them.
Fortunately, real complications are rare following properly performed dimple surgery. In practice, the vast majority of interventions go without any problems and patients are fully satisfied with their results.
However, and despite their rarity, you must be informed of the possible complications:
- Bleeding: it is possible for the first few hours but usually remains very moderate. When they are too large, this may justify further drilling, or even a return to the operating room.
- Hematomas: they may require evacuation if they are large or too painful.
- Infection: despite the natural presence of microbes in the nasal passages, it is very rare. If necessary, it quickly warrants appropriate treatment.
- Unsightly scars: they can only concern external scars (when they exist) and are very rarely unsightly to the point of requiring a touch-up.
- Skin damage: although rare, they are still possible, often due to nasal splints or tobacco. Simple wounds or erosions heal spontaneously without leaving any traces, unlike skin necrosis, fortunately exceptional, which often leaves a small area of scarred skin.
- The symmetry
- An inflammatory reaction on the suture
- A nerve injury (buccal branch of the facial nerve)
- A lesion of the salivary gland duct (Stenon’s duct)
Overall, we must not overestimate the risks, but simply be aware that a surgical intervention, even apparently simple, always involves a small amount of hazards. Using a qualified Plastic Surgeon assures you that he or she has the training and skills required to know how to avoid these complications, or treat them effectively if necessary.
